IMPORTANT INSURANCE NOTES
Insurance enrollment information must be returned to Human Resources within 31 days of hire date or of the date of the full time notification letter. After that period, enrollments will be done only during the Benefits Fair held in March of each year except in the instance of a Late Enrollee or Special Enrollee as specified in the Health Insurance Summary Plan.
All health insurance plans offered by South Central have a 12 month pre-existing clause unless a Certificate of Insurance from a previous employer is submitted at the time of enrollment per Federal regulations. When enrolling as a Late Enrollee, where there was no prior insurance coverage, the pre-existing period will be 18 months. Pre-existing refers to anything you or your dependents have been treated for prior to your eligibility date and will not be covered for the first 12 months you are on the insurance. All pre-existing conditions are determined by information obtained by the insurance company from your attending physician.
Plans I and II require a pre-admission certification for non-employers hospitals. (See attached Schedule of Benefits for pre-cert information and telephone number along with instructions for approval of extended hospital confinement.) Please note that pre-certification only certifies medical necessity of procedure or hospital stay and does not guarantee payment of benefits. All benefits and exclusions are determined by the insurance company according to the plan document.
Failure to provide birth dates and social security numbers of dependents could cause a delay in receiving insurance ID Cards and Booklets.
